Isolation Tag is normally two part perforated and consists of two identical sections which are completed by the person in charge.

The top half is attached to the isolation point, the bottom stapled to the Permit to Work. The isolation is deemed to be removed when both halves are re-attached to each other.

By using the perforation on the tag, each section can easily be removed without the need for either scissors or knives which may require additional permits.
